Medical fitness certificate
Proof of good health for a job, school enrolment or a general fitness check. Choose Medical Fitness Certificate on the form.
People in the Philippines can request an online fitness certificate from DrCertificate, a platform that connects you with independent certified doctors. Each request goes to a doctor who reviews your details through a digital chat consultation.
Issuing a certificate is never automatic.
The doctor decides whether a certificate is appropriate. An approved certificate arrives as a PDF by email, and you can start with the online form whenever you are ready.
Anyone in the Philippines aged 16 or over can request an online fitness certificate from DrCertificate. You fill in the online form, pay 399 pesos through Stripe, and an independent certified doctor reviews the request by digital chat. If the doctor approves it, the certificate arrives by email as a PDF. The service covers suitable non emergency situations only.
Workers, students and travellers are the three groups in the Philippines who most often need a medical certificate. A new job, a school enrolment or a flight can each come with a request for proof of health.
New hires in Makati, BGC or Cebu IT Park are often asked for a fit to work note before their start date. Students may need a good health certificate for enrolment or to join a school sports team.
Weather plays a part too.
The rainy season, roughly June to November, brings typhoons, flu and dengue worries, and plenty of people end up needing a day off and a note to go with it. Travellers passing through Ninoy Aquino International Airport in Manila, Mactan Cebu International Airport or Francisco Bangoy International Airport in Davao sometimes need a fitness note before flying.
Others need proof that illness stopped them from boarding a booked flight. A clinic queue is hard to fit around a shift or a departure time, and an online request can be sent from home or a hotel room.
Whether the note is accepted is up to the employer, school or airline. Check with them first.
Four options appear on the DrCertificate form: Medical Fitness Certificate, Sick Leave, Unfit to Travel and Work From Home
Pick the option that matches what your employer, school, airline or sports body asked for. Some needs share one form option, as the cards show.
Proof of good health for a job, school enrolment or a general fitness check. Choose Medical Fitness Certificate on the form.
For a gym, team, race or league that wants a fitness note before you take part. Uses the Medical Fitness Certificate option.
For an airline or employer that wants confirmation you are well enough to travel or start work. Also uses the Medical Fitness Certificate option.
For time off work or university when you are unwell. A general medical certificate uses the Sick Leave option too.
For when illness stops you from travelling on a booked date. Choose Unfit to Travel on the form.
For when you can still work but should stay home while unwell. Choose Work From Home on the form.
Getting an online fitness certificate in the Philippines takes four steps, all done online. DrCertificate has no video calls and no in person consultations.
The whole exchange with the doctor happens in a digital chat.

Enter your details, choose the certificate option that fits and describe why you need it and for which dates.
Pay the 399 peso fee through the Stripe checkout to send your request for review.
An independent certified doctor reads your request in a digital chat and may ask follow-up questions. Reply quickly to avoid delays.
If the doctor approves, the certificate arrives by email as a PDF. If the doctor finds it inappropriate, the request is declined.

Have your personal details, dates and any requested wording ready before you open the form. A complete request is quicker for the doctor to review.
Make sure the name you enter matches your ID as written.
Your full name as shown on your ID, your date of birth and an email address you can check right away.
Why you need the certificate and the dates it should cover.
Any form, template or wording your employer, school, airline or sports body asked for.
A clear account of current symptoms, or existing medical records that support a fitness request.
The doctor reads everything you submitted and decides independently whether a certificate is appropriate. Paying the fee buys a review, not a guaranteed certificate.
The doctor checks that your name and ID match, that the dates make sense and that the symptoms or fitness details you shared are consistent with each other.
The doctor may ask follow-up questions in the chat.
DrCertificate is a platform that connects you with independent doctors. The platform does not diagnose, treat or prescribe, and the review covers the certificate request alone. If what you describe calls for an examination or treatment, the doctor can decline, and a visit to a clinic becomes the better route.
A certificate is issued only when the doctor finds it appropriate based on what you shared. The decision rests with the doctor every time, and neither the fee nor the form can change that.
If the doctor declines, you do not receive a certificate and the fee is not refunded. Doctors decline about 2% of requests, when the name and ID do not match or when the symptoms and details shared are inconsistent.
Payments are non refundable. The one exception is a verified technical fault reported within 24 hours.
Your next step is a local clinic or hospital, where a doctor can examine you in person and issue a note if appropriate. For help with a request, email care@drcertificate.com.

Turnaround for a DrCertificate request is about 30 minutes. Missing details, a name that does not match your ID or follow up questions from the doctor can add time.
Fitness cases are reviewed during set hours in India time, which runs two and a half hours behind the Philippines. A request sent outside those hours waits for the next review window.
| Day | India time | Philippine time |
|---|---|---|
| Monday to Saturday | 9 am to 9 pm | 11:30 am to 11:30 pm |
| Sunday | 10 am to 8 pm | 12:30 pm to 10:30 pm |
Acceptance is decided by the organisation receiving the certificate, not by DrCertificate or the doctor. Ask your employer, school, airline or sports body whether it takes online medical notes before you apply.
Company policy or a school handbook usually sets when a sick leave certificate is needed. Fit to fly and unfit to fly needs differ by airline, so read your carrier's own rules.
Two official processes have their own route. Land Transportation Office Administrative Order AVT-2015-026 makes a medical certificate stating the applicant is physically and mentally fit to operate a motor vehicle a prior requirement for issuing or renewing a driver's licence. The same order defines an accredited physician as a practising doctor licensed to practise medicine in the Philippines, and the certificate must show the physician's Professional Regulation Commission number.
Republic Act 10022 has the Department of Health regulate clinics that conduct health examinations for overseas employment, and gives each migrant worker the freedom to choose any DOH accredited or DOH operated clinic.
For a driver's licence or an overseas job medical, use those channels.
An online certificate is not right for emergencies. Call 911, the national emergency hotline in the Philippines, or go to the nearest hospital for chest pain, trouble breathing, heavy bleeding or a serious injury.
DrCertificate certificates are not for legal purposes, insurance claims or submission in India. Applicants must be 16 or older.
Visit a local clinic or hospital in person if your symptoms are getting worse, if you need a test, treatment or a prescription, or if the organisation asking for the certificate wants a physical examination. A doctor who can see and examine you is the better choice in those cases.
DrCertificate explains how the service works before you pay, so you know what to expect.
All doctors on the platform are registered with a medical council and hold an MBBS, MD or MS.
Sports fitness, fit to fly and fit to work certificates use the Medical Fitness Certificate option. A general medical certificate uses the Sick Leave option.
Doctors decline about 2% of requests, when the name and ID do not match or the symptoms and details shared are inconsistent.
In most South East Asian countries, sick leave and work from home notes go up to 7 days for generic symptoms, and fitness notes are generally valid for 30 days.
Customers are encouraged to check that the place of submission accepts online medical notes before choosing the service.
No video call or clinic visit is needed. DrCertificate works through an online form and a digital chat with an independent certified doctor. If the doctor decides your situation needs a physical examination, the request is declined, and a local clinic or hospital in the Philippines is the right next step.
Whether a certificate can cover dates before your consultation is the doctor's decision alone. Explain the dates and the reason accurately on the form. The doctor may approve or decline, and paying the 399 peso fee does not guarantee that earlier dates will be included.
Your details go to the reviewing doctor so the doctor can assess the request and prepare the certificate if approved. Stripe handles the payment. For questions about how DrCertificate stores or uses your information, email care@drcertificate.com before you apply.
Email care@drcertificate.com for help with a request, a payment or a technical problem. A verified technical fault must be reported within 24 hours to qualify for a refund. The DrCertificate address is KMC Armstrong Corporate Center, H.V. Dela Costa, Salcedo Village, Makati City, 1227 Metro Manila.
A fit to fly request uses the Medical Fitness Certificate option, and the airline decides whether it accepts an online note. Check with your carrier before flying out of Ninoy Aquino, Mactan Cebu or Davao. Apply early so the doctor has time to review before your departure.
Fitness notes are generally valid for 30 days in most South East Asian countries, and sick leave or work from home notes go up to 7 days for generic symptoms. The organisation receiving your certificate may set its own rules, so confirm its validity window first.
No. A driver's licence medical certificate must come from an accredited physician licensed in the Philippines, showing a PRC number, under LTO Administrative Order AVT-2015-026. Overseas employment examinations take place at DOH accredited or DOH operated clinics under Republic Act 10022.
