Medical fitness certificate
Confirms general good health for a new job, a course or school enrolment. Choose Medical Fitness Certificate.
DrCertificate lets you request an online fitness certificate in Thailand without visiting a clinic. You fill in a short form, and an independent certified doctor reviews your request after a digital chat consultation.
Issue is never automatic.
The doctor decides whether a medical fitness certificate is appropriate, based on what you share. If approved, the certificate arrives as a PDF by email. The service suits non emergency needs for workers, students, travellers and sports participants across Thailand.
Yes. Anyone in Thailand aged 16 or over can request an online fitness certificate through DrCertificate. You complete the online form, pay through Stripe, and an independent certified doctor reviews your request in a digital chat. If the doctor approves it, the certificate arrives as a PDF by email. The service is for suitable non emergency situations only.
Workers, students and travellers in Thailand are the groups who most often need a medical fitness certificate. A new employer may want proof of good health before a start date, a university may ask for one at enrolment, and a sports club may want one before a race or season.
Thailand has its own situations too.
Foreign staff applying for a work permit through the Board of Investment's One Start One Stop centre are asked for a medical certificate issued within the last 6 months, confirming no prohibited diseases under Ministerial Regulations. Foreigners living in Thailand who convert an overseas car or motorcycle licence to a Thai one must show a health certificate from a certified medical professional.
Both needs can involve tests or an in person exam. Confirm with the office what it requires before choosing an online note. A short email to the right desk can save a wasted fee.
Weather plays a part as well. The rainy season, roughly May to October, brings colds, flu and dengue, and a fitness note after a short illness can help someone return to work or training. Travellers passing through Suvarnabhumi or Don Mueang airports in Bangkok may need a fit to fly note for an airline.
An online request lets you start from home, a hotel or an office in Sukhumvit, Silom or Chiang Mai. Whether the receiving organisation accepts it is that organisation's decision.
Four options on the form cover every certificate listed here
Most people in Thailand looking for a fitness certificate should choose Medical Fitness Certificate on the form. The form has four options: Sick Leave, Unfit to Travel, Work From Home and Medical Fitness Certificate.
Sports fitness, fit to fly and fit to work certificates all sit under Medical Fitness Certificate.

Have your personal details, a working email address, your reason and dates, and any requested wording ready before you start.
Details that match your ID come first. A name and ID mismatch is a common reason a doctor declines.
Your full name and ID details as they appear on your documents, plus an email address you check.
Why you need the certificate and the start and end dates it should cover.
Any form or set wording your employer, school, airline or sports club asked for.
Current symptoms, relevant history and any existing medical records.
The doctor reads everything you submitted and decides whether a fitness certificate is appropriate. Your stated reason, the dates, your symptoms or health history, and whether your name and ID match all form part of that review.
Questions can come up.
If something needs clarifying, the doctor may ask follow-up questions in the digital chat. Quick, honest answers keep the review moving. DrCertificate itself does not diagnose, treat or prescribe. The platform connects you with independent doctors who hold MBBS, MD or MS qualifications. The doctor's judgement is what counts.
Paying the fee pays for that review and does not guarantee a certificate, which is issued only when the doctor finds it appropriate.
If the doctor declines, you do not receive a certificate and the fee is not refunded. Doctors decline about 2 percent of requests, usually when the name and ID do not match or the symptoms and details shared are inconsistent.
The doctor may also feel your situation needs a physical exam. In that case, visit a clinic or hospital near you in Thailand.
Refunds apply only for a verified technical fault reported within 24 hours. For help, email care@drcertificate.com.
The fee depends on the country you apply from, and the amount for Thailand appears at Stripe checkout before you pay. The table shows fees confirmed for other countries in the region.
The fee covers the doctor's review and, if approved, the PDF certificate by email. Payment goes through Stripe.
The fee is non refundable, including when the doctor declines, unless a verified technical fault is reported within 24 hours.

Acceptance is decided by the organisation receiving the certificate. Ask your employer, school, airline or sports body first whether it takes an online medical note.
Thai rules differ by purpose. Under the Labour Protection Act, an employer can ask for a medical certificate when sick leave lasts three working days or more, and the law refers to a first class modern medicine practitioner or an official medical facility. Employees working with risk factors face a stricter standard.
A 2020 Ministerial Regulation requires a first health exam within 30 days of hiring and at least one each year after that, by a doctor certified or trained in occupational medicine. An online fitness certificate does not replace that exam.
Airlines set their own rules for fit to fly and unfit to fly notes, and those rules can differ by carrier.
Get the wording you need in writing. DrCertificate encourages every applicant to confirm acceptance before applying.
An online certificate is not right for emergencies, legal matters, insurance claims or anyone under 16. In a medical emergency in Thailand, call 1669 or go to the nearest hospital.
The certificate is not for legal purposes, must not be used for insurance, and is not for submission in India.
Choose an in person visit at a clinic or hospital when you need a physical exam, blood or other tests, an occupational health check, or a work permit certificate confirming no prohibited diseases. The same applies if your symptoms are severe or getting worse.
DrCertificate is an online platform run by Health Venture OU that connects people in ten South East Asian countries, Thailand included, with independent doctors. These reasons explain how it works in practice.
All doctors on the platform are registered with a medical council and hold MBBS, MD or MS qualifications.
DrCertificate encourages customers to check that the place of submission accepts online medical notes before choosing the service.
Sports fitness, fit to fly and fit to work certificates use the Medical Fitness Certificate option. A general medical certificate uses the Sick Leave option.
Doctors decline about 2 percent of requests, when details are wrong or the information shared is inconsistent.
In most South East Asian countries, fitness notes are generally valid for 30 days, and sick leave and work from home notes go up to 7 days for generic symptoms.
No. DrCertificate works through an online form and a digital chat with an independent certified doctor, with no video or in person consultation. If the doctor decides your situation needs a physical exam or tests, a clinic or hospital in Thailand is the better route.
Only if the doctor agrees. The doctor decides which dates a certificate can cover, based on the information you share. Fitness notes are generally valid for 30 days, so apply close to the date your employer, school or club needs the certificate.
Your details go to the doctor reviewing your request and are used to assess it and prepare the certificate if approved. Card payment is handled at Stripe checkout. Send any privacy question to care@drcertificate.com before you apply.
Email care@drcertificate.com with questions about form options, delivery or technical faults. A verified technical fault must be reported within 24 hours to qualify for a refund. The business address is KMC Armstrong Corporate Center, Salcedo Village, Makati City, Metro Manila, Philippines.
You can request one by choosing Medical Fitness Certificate and stating your travel date and airline. The doctor decides whether to issue it. Airlines set their own rules, so check with your carrier and apply well before your flight, within the doctors' hours.
That depends on the office handling your case. Work permits need a certificate from the last 6 months confirming no prohibited diseases, and licence conversion needs one from a certified medical professional. Ask the office first, since tests may be required.
Start the online form once you know a fitness certificate fits your need and the receiving organisation accepts online notes. A certified doctor decides whether to issue it, and fees are non refundable. Questions first? Email care@drcertificate.com.
