Medical fitness certificate format: what you need to know and how to get one
Have you been asked to supply a health clearance document and feel unsure about its layout? This guide explains the medical fitness certificate format most organisations expect, what each part means and how to request one online.
DrCertificate connects people in Singapore, Malaysia, the Philippines and seven other South East Asian countries with independent registered doctors. A doctor reviews every request after a digital chat consultation and decides whether to issue a certificate.
The short answer on medical fitness certificate format
A medical fitness certificate format usually shows your full name, the date of review, a clear statement that you are fit for a stated purpose, and the doctor's name, qualification and signature. On DrCertificate, an independent registered doctor reviews your request in a chat and decides whether to issue one. Your organisation's own format rules come first.

What are the main facts about a medical fitness certificate?
No single official format
There is no one official medical fitness certificate format across South East Asia. The receiving organisation sets its own rules.
The doctor decides
An independent doctor with an MBBS, MD or MS decides whether a certificate is issued and what it states.
Usual validity
In most South East Asian countries, fitness notes are generally valid for 30 days.
Delivery by email
Approved DrCertificate certificates arrive by email as a PDF, usually in about 30 minutes.
What is a medical fitness certificate?
A medical fitness certificate is a short document in which a doctor states that, after reviewing your health details, you are fit for a stated purpose like starting a job, joining a course or taking part in sport. The certificate records the doctor's opinion on the day of the review.
A fitness certificate does not list a diagnosis, give treatment advice or stand in for a full examination with tests.
On DrCertificate, the matching choice is the Medical Fitness Certificate option on the application form. Sports fitness, fit to fly and fit to work requests use that same option.
People often mix it up with a sick leave certificate. A sick leave certificate states that you were ill and required recuperation over a fixed length of time, while a fitness certificate says you are fit now. A general medical certificate is broader and, on DrCertificate, goes through the Sick Leave option.
When do you need a medical fitness certificate?
You need a medical fitness certificate when an employer, school, sports organiser or travel provider wants a doctor's confirmation that you are fit for what comes next.
Common moments include starting a new job, enrolling in a course, joining a team and returning to duties after time away. HR teams, school offices, coaches and airline staff are the people who usually ask.
The reason is simple. The organisation wants to know you can take part safely, and it wants a record that a doctor looked at the question.
Two official examples show how this works in the region. Singapore's Ministry of Manpower says that if a medical examination shows an employee previously removed from exposure is fit for hazardous work, a designated workplace doctor needs to submit a certificate of fitness before that employee can return.
In the Philippines, the Omnibus Rules, which carry out the Labor Code, say the physician engaged by an employer under the rule shall conduct a free medical examination before employment, for the proper selection and placement of workers, plus a free annual physical examination.
Those are regulated workplace checks done by doctors named in the rules. An online certificate does not replace them.
Medical fitness certificate format at a glance
The medical fitness certificate format varies by organisation, but the same facts apply to most requests. The doctor decides the outcome in every case.
| Item | What to expect |
|---|---|
| Typical situations | A new job, course enrolment, sports or travel |
| What the doctor considers | Your form answers, health history, current symptoms and the purpose you state |
| What the certificate covers | A fitness statement for the stated purpose, dated on the day of review |
| Fitness outcome | Decided by the doctor case by case, and a request can be declined |
| Usual validity | Fitness notes are generally valid for 30 days in most South East Asian countries |
| Form option on DrCertificate | Medical Fitness Certificate |
| When an in person visit suits better | When the organisation wants tests, a physical exam or a named clinic, or when you have symptoms during exercise or a known heart condition |
Who issues a medical fitness certificate?
Independent doctors with MBBS, MD or MS qualifications review requests and issue every DrCertificate medical fitness certificate. Each doctor is registered with a medical council, and the doctor alone decides whether a certificate is issued.
DrCertificate is the platform in the middle.
The platform connects applicants with doctors, hosts the form, takes payment through Stripe and sends approved PDFs by email. DrCertificate does not diagnose, treat or prescribe. When a request arrives, the doctor reads your answers and asks questions in the chat if anything is unclear. The doctor then decides based on what you shared. Nobody on the platform side makes that medical call.
Before you apply, check that the place where you will submit the certificate accepts online medical notes.
How do you get a medical fitness certificate online, step by step?
Getting a medical fitness certificate through DrCertificate takes five steps, all online. There are no video or in person consultations at any stage.

အဆင့် 1: Choose the right option
Open the application form and pick the Medical Fitness Certificate option, which covers employee fitness, student fitness and good health certificates. Sports, fit to fly and fit to work requests use this option too.
အဆင့် 2: Fill in the form accurately
Enter your name as it is written on your ID, the purpose of the certificate and your health details. Mismatched names or inconsistent answers are the usual reasons a doctor declines.
အဆင့် 3: Pay through Stripe
Payment is made through Stripe. The price depends on your country, and payments are non refundable.
Step 4: Chat with the doctor
An independent doctor reviews your request in a digital chat, may ask follow-up questions and then decides whether to issue a certificate.
Step 5: Receive the PDF by email
If the doctor approves, the certificate arrives by email as a PDF, usually in about 30 minutes.

What should a medical fitness certificate include?
A medical fitness certificate should include four things: who you are, when the doctor reviewed you, what you are fit for and who issued it. Without any one of those, HR or a school office may send it back.
Most formats show the details listed below.
Before you submit, run a quick check. Does your name match your ID letter for letter? Is the date recent enough for the organisation? Does the stated purpose match what was asked, for example a job rather than a sport?
Ask your organisation for its own format first. Employers and universities sometimes hand out their own form or want extra details like vitals or test results, and a short online review cannot include tests.
Full name
Your name as written on your ID.
Date
The date of the doctor's review.
Fitness statement
A plain line saying you were found fit.
Purpose
What the certificate is for, like work, study or sport.
Doctor details
The doctor's name, qualification and signature.
Will your employer or school accept a medical fitness certificate?
Your employer, school or airline decides whether to accept a medical fitness certificate. Neither DrCertificate nor the doctor can promise acceptance, which is why checking first matters.
Organisations question certificates for a handful of reasons. A frequent one is the wrong type, like a sick leave note sent when a fitness certificate was requested. Others include a name that does not match records, a missing purpose or a date outside the window the organisation allows. An organisation may also insist that only its own doctor or clinic signs.
If yours is questioned, ask what is missing and whether the organisation accepts online medical notes at all. If it needs its own form, tests or a named clinic, an in person visit is the better route. A certificate cannot be corrected once issued unless the fault came from the medical team, so get the details right on the form.
Check before you pay. It saves money and time.
When is an online medical fitness certificate not the right choice?
The service is only for suitable non emergency situations. Chest pain, trouble breathing, fainting or any sudden severe symptom needs in person or emergency care, not a chat.
The doctor may also decline to issue a certificate. That happens when details are wrong, like a name and ID that do not match, or when the symptoms and details shared are inconsistent.
DrCertificate certificates are not for submission in India and not for legal purposes. DrCertificate certificates must not be used for insurance either.
Applicants must be at least 16. Recovery and caretaker certificates are not offered.

Frequently asked questions about medical fitness certificate format
No. A medical fitness certificate describes your fitness at the time the doctor reviews your request, so it should not carry an earlier date. If your organisation needs a document covering a past date, ask what it will accept before you apply, because the doctor decides what the certificate states.
The doctor decides, case by case. In most South East Asian countries, fitness notes are generally valid for 30 days, while sick leave and work from home notes go up to 7 days for generic symptoms. Your employer, school or airline may also set its own validity window.
Yes, through a digital chat. The independent doctor reads your form and may ask follow up questions in the chat before deciding. DrCertificate does not offer video or in person consultations, so the chat is the only contact you have with the doctor during the review.
If the doctor declines, no certificate is issued and the payment is not refunded. Doctors decline about 2% of requests when details are wrong, like a name that does not match the ID, or when the symptoms and details shared are inconsistent with each other.
No. There is no single medical fitness certificate format used across South East Asia. Each employer, school, sports body or airline decides what it accepts and may hand out its own form. The details most formats share are your name, the review date, a fitness statement and the doctor's details.
Only if the mistake came from the medical team. Otherwise a DrCertificate certificate cannot be corrected once issued, so check your name, ID details and purpose carefully on the form before you pay. A typo you entered yourself stays on the certificate.
