What is a medical certificate? What you need to know and how to get one

A medical certificate is a document from a doctor that records your health status for a stated purpose, from sick leave to fitness checks. This guide explains what a medical certificate says, when you need one and what it should include.

DrCertificate connects people in Singapore, Malaysia, the Philippines and seven other South East Asian countries with independent registered doctors. A doctor reviews every request after a digital chat consultation and decides whether to issue a certificate.

The short answer on what a medical certificate is

A medical certificate is a document a registered doctor prepares after assessing you, stating medical information or an opinion for a stated reason, like sick leave or fitness. On DrCertificate, an independent doctor reviews your online request and decides whether to issue one. The receiving employer, school or airline decides whether to accept it.

Medical certificate facts in brief

  • Purpose comes first

A medical certificate records a doctor's opinion for one stated purpose, so a sick leave note and a fitness note are not interchangeable.

  • Doctors decide

Only a qualified, registered doctor decides whether a medical certificate is issued and what it covers.

  • Typical lengths

In most South East Asian countries, sick leave and work from home notes go up to 7 days for generic symptoms, and fitness notes are generally valid for 30 days.

  • The receiver has the last word

Your employer, school or airline decides whether to accept a medical certificate, so check its rules before you apply.

What is a medical certificate?

After examining a patient, a doctor writes a medical certificate, a record that sets out medical details or the doctor's view on a specified matter. It tells an employer, school or airline what the doctor concluded about your health on a given date.

A medical certificate states only what the doctor can stand behind, like whether you were unwell, how long rest was advised or whether you were fit for an activity. It does not prove a diagnosis in court, replace treatment or work as an insurance document.

The type matters more than the name.

A sick leave certificate records that an employee or learner is unwell and requires time off for health reasons. A medical fitness certificate verifies that someone has undergone evaluation and found suitable for a specified role, such as employment, athletics or air travel. Work from home and unfit to travel certificates cover the cases in between.

On DrCertificate, a general medical certificate uses the Sick Leave option, while sports, fit to fly and fit to work notes use the Medical Fitness Certificate option.

When do you need a medical certificate?

  • You need a medical certificate when an organisation asks for a doctor's written opinion before it grants leave, excuses an absence or lets you take part in an activity. Work, study and travel are the usual triggers.

  • At work, HR teams ask for one when you take sick leave or start a new role. In Singapore, the Ministry of Manpower says an employee must be certified unfit for work by a medical practitioner registered under the Medical Registration Act or Dental Registration Act to qualify for paid sick leave.

  • Universities and schools ask for a medical certificate to excuse missed classes, exams or deadlines. Student offices want proof the absence had a medical reason.

  • Travel works differently. An airline may ask for a fit to fly note before carrying a passenger who has been ill recently, or for an unfit to travel note when a passenger wants to change plans for health reasons. Sports clubs and race organisers ask for fitness notes before events.

  • Each of these bodies wants a doctor, not a desk clerk, to make the health call. The paper gives them a record they can file if questions come up later.

  • Why ask at all? A certificate moves the health judgement to a qualified doctor and keeps it away from a manager or teacher.

Medical certificate types at a glance

The doctor decides the number of days or the fitness outcome case by case. An in person visit suits anything that needs a physical exam, tests or urgent care.

Who issues a medical certificate?

Only a suitably trained and licensed physician who is entitled to carry out the relevant evaluation ought to sign a medical certificate. On DrCertificate, that means an independent doctor holding an MBBS, MD or MS qualification.

DrCertificate is a platform, not a clinic. It connects applicants with doctors and handles the form and payment. The platform does not diagnose, treat or prescribe.

The doctor makes the medical call.

Every doctor on the platform is registered with a medical council. The doctor reads your form, chats with you where needed and decides whether a certificate is justified and what it should say. The platform team does not make that decision.

Registration matters because it lets an employer or school trace the certificate to an accountable professional. If your organisation insists on a doctor registered in your own country, check that before you apply.

How do you get a medical certificate online, step by step?

You get a medical certificate online through DrCertificate by filling in a form, paying, and waiting for a doctor's decision by email. The whole route is text based.

There are no video or in person consultations.

  1. Step 1: Pick the right form option

Choose Sick Leave, Work From Home, Unfit to Travel or Medical Fitness Certificate. Fit to fly, sports and fit to work requests go under Medical Fitness Certificate.

  2. Step 2: Fill in the form accurately

Enter your name as shown on your ID, plus your symptoms or the purpose of the certificate. A name that does not match the ID, or details that conflict, are the usual reasons a doctor declines.

  3. Step 3: Pay through Stripe

Payment is taken through Stripe when you submit. The fee is non refundable, so check your details before paying.

  4. Step 4: Chat with the doctor

An independent doctor reviews your request in a digital chat, may ask follow up questions and decides whether to issue a certificate.

  5. Step 5: Receive your PDF

Approved certificates arrive by email as a PDF, usually in about 30 minutes.

What should a medical certificate include?

A medical certificate should show who it is for, which dates it covers, what it is for and which doctor issued it. Missing any of those gives the receiver a reason to send it back.

Most certificates carry your full name and the dates of assessment and issue. They also show the period of rest or the fitness outcome, plus the doctor's name, qualification, registration details and signature.

Ask your HR team, school office or airline whether they use their own form. Find out before you apply, because a certificate cannot be corrected once issued unless the fault came from the medical team.

Run this check before you submit.

  • Name matches your ID

Spelling and order match the ID your employer or school holds.

  • Dates are right

Start and end dates match the leave you are requesting.

  • Correct type

A sick leave note for leave, a fitness note for fitness.

  • Doctor details shown

Name, qualification and registration details are present.

  • Format accepted

The receiver accepts online certificates in this format.

Will your employer or school accept a medical certificate?

Your employer, school or airline decides whether to accept a medical certificate, not the doctor and not DrCertificate. Most organisations set their own rules on who can issue a certificate and in what format.

DrCertificate encourages every customer to check that the place of submission accepts online medical notes before choosing the service. A short email to HR or the student office saves money and stress.

Certificates get questioned for ordinary reasons. One frequent problem is the wrong type, like a sick leave note handed in where a fitness note was needed. Dates that do not match the leave request cause trouble too. So does a name that differs from company records. An organisation that insists on its own printed form will also turn away anything else.

If your certificate is questioned, ask the reviewer what is missing. You can point to the doctor's name and registration details on the PDF. If the receiver needs a physical exam or a doctor registered in your own country, a local clinic is the better route.

One more thing to remember.

DrCertificate cannot change an issued certificate unless the error came from the medical team.

When is an online medical certificate not the right choice?

An online medical certificate is not the right choice for emergencies, legal matters, insurance or anyone under 16. The doctor can also decline any request that does not fit.

If you have chest pain, trouble breathing, a high fever that will not settle or a symptom that is getting worse fast, go to a clinic or emergency department straight away. DrCertificate is only for suitable non emergency situations.

DrCertificate certificates are not for submission in India, not for legal purposes and must not be used for insurance. Applicants must be at least 16. Recovery and caretaker certificates are not offered.

Doctors decline a small share of requests. That happens when details are wrong, like a name that does not match the ID, or when symptoms and other details do not add up. A declined request is not refunded.
