Sick leave or fitness certificate: what you need to know and how to get one

Not sure whether you need a sick leave or fitness certificate? A sick leave certificate says you were unwell and unfit for work or study on stated dates. A fitness certificate says a doctor found you fit for work, study, sport or travel.

DrCertificate connects people in 10 South East Asian countries with independent registered doctors, who review each request after a digital chat consultation and decide whether to issue a certificate.

What is a sick leave or fitness certificate?

A sick leave certificate is a doctor's statement that you were unwell and unfit for work or study on stated dates. A fitness certificate is a doctor's statement that you are fit for a stated purpose, like starting a job, joining a sports club or flying.

Neither certificate lists your full medical history.

A sick leave note does not say you are fit to return. A fitness note does not excuse past absence.

On the DrCertificate form, choose the Sick Leave option for university or work if you need time off covered. A general medical certificate uses that same option. Choose the Medical Fitness Certificate option for employee, student, sports, fit to work or fit to fly needs.

Two lookalikes cause confusion. A work from home certificate says you can work but should stay home, and an unfit to travel certificate says you should not fly. Both have their own options on the form.

When do you need a sick leave or fitness certificate?

  • You need a sick leave certificate when an employer, university or school asks you to prove an absence was due to illness. You need a fitness certificate when they want proof you are well enough to start or continue something. The person asking is usually HR, a school office, a coach or an airline.

  • Sick leave notes come up after a missed shift, a missed exam or a run of days off with fever, stomach upset or a migraine. HR asks so the days count as paid sick leave instead of unpaid or annual leave. Universities ask because absences can affect attendance records and exam eligibility.

  • Fitness certificates come earlier in the story.

A new employer may want one before your start date. A school may ask before a trip or a sports day. A gym, club or race organiser wants proof you can train safely. An airline may ask for a fit to fly note after a recent illness, and the airline still makes its own boarding decision.

  • Every one of these organisations wants a doctor's written opinion so nobody in the office has to make a medical call.

  • Two signals tell you which one you need. If the question is about the past, it is sick leave. If it is about what comes next, it is fitness.

Who issues a sick leave or fitness certificate?

A sick leave or fitness certificate on DrCertificate is issued only by an independent doctor with an MBBS, MD or MS qualification. All doctors on the platform are registered with a medical council. The doctor reads your request, asks questions in a digital chat and makes the medical decision.

DrCertificate is the platform in the middle. DrCertificate connects applicants with doctors, runs the form and payment, and sends approved certificates by email. DrCertificate does not diagnose, treat or prescribe.

That split matters.

When HR or a school checks a certificate, the question is whether a qualified, registered doctor stands behind it. Rules on who may certify differ by country. In Singapore, the Ministry of Manpower says an employee qualifies for paid sick leave only when certified unfit for work by a medical practitioner registered under the Medical Registration Act or Dental Registration Act.

Check what your own country and employer require first. DrCertificate encourages every customer to confirm that the place of submission accepts online medical notes before choosing the service.

How do you get a sick leave or fitness certificate online, step by step?

Getting a sick leave or fitness certificate on DrCertificate takes five steps, all online. There are no video calls and no in person visits, only the form and a digital chat with the doctor.

  1. ជំហានទី 1៖ Choose the right option

Open the online form and pick the Sick Leave option for university or work if you were unwell, or the Medical Fitness Certificate option for work, study, sport or fit to fly needs.

  2. ជំហានទី 2៖ Fill in the form with honest details

Enter your name and ID just as they appear on your documents and describe your symptoms or purpose accurately. Doctors decline about 2% of requests when the name and ID do not match or the details shared are inconsistent.

  3. ជំហានទី 3៖ Pay through Stripe

Pay the fee for your country through Stripe. The payment is non refundable, including when the doctor does not issue a certificate.

  4. Step 4: Chat with the doctor

An independent registered doctor reviews your request in a digital chat, may ask follow-up questions and decides whether to issue a certificate and what it covers.

  5. Step 5: Receive your PDF by email

If the doctor approves the request, your certificate arrives by email as a PDF, usually in about 30 minutes.

What should a sick leave or fitness certificate include?

A sick leave or fitness certificate usually shows your full name, the consultation date, the purpose and the doctor's name, qualification and registration details. A sick leave note adds the days you were unfit. A fitness note states what you were found fit for.

Run a short check before you send the PDF.

Ask the organisation about its own format first. An employer may want the file uploaded to an HR portal, and a sports club may insist on its own form. Knowing this before you apply saves trouble, because a certificate cannot be corrected once issued unless the fault came from the medical team.

  • Name and ID

Your name and any ID match your official records.

  • Dates

The dates or validity period line up with your absence or your event.

  • Purpose

The certificate type and purpose match what the organisation asked for.

  • Doctor details

The doctor's name, qualification and registration details are visible.

Will your employer or school accept a sick leave or fitness certificate?

Acceptance of a sick leave or fitness certificate is decided by the employer, school or airline that receives it, not by DrCertificate or the doctor. Each organisation sets its own rules on which notes it takes. Ask before you apply, not after.

A certificate gets questioned for a handful of common reasons. The type may be wrong, like a sick leave note sent when HR wanted a fit to work note. Details may be missing or mismatched, or the dates may not match the absence. A company policy may require a doctor from an approved panel. A role may need a physical exam that no online chat can replace.

In the Philippines, the Omnibus Rules that carry out the Labor Code say the physician engaged by an employer under the rule conducts a free pre employment medical examination and a free annual physical examination. An online note does not stand in for that.

If your certificate is questioned, ask the organisation to tell you in writing what it needs. Show the doctor's details printed on the certificate and check that you picked the right form option. If an in person exam is required, book one with a local clinic.

When is an online sick leave or fitness certificate not the right choice?

An online sick leave or fitness certificate is not the right choice when your symptoms are urgent, severe or getting worse. Chest pain, trouble breathing, heavy bleeding or a high fever that will not settle need in person or emergency care straight away.

DrCertificate is for suitable non emergency situations only. The doctor may decline any request, and the payment is not refunded when that happens. If you have been ill for more than a week, a doctor who can examine you is the better route.

Other limits apply too.

  • Certificates are not for submission in India.
  • Not for legal or insurance use Certificates are not for legal purposes and must not be used for insurance.
  • Age 16 and over Applicants must be at least 16 years old.
  • Recovery and caretaker notes Recovery and caretaker certificates are not offered.
